Use the current form

Check the current Leasehold 3 form and T541 guidance on GOV.UK immediately before filing. As checked on 18 August 2026, the service-charge application fee is £114 and the hearing fee, where a hearing is held, is £227. Fees change: verify rather than copying these figures into a future application.

Describe the premises, parties, lease and charge years precisely. Attach only the documents needed to understand the application at this stage unless the form or directions require more.

  • Lease and relevant variations
  • Demand/accounts for each year
  • Short statement of case
  • Issue schedule
  • Fee remission check if relevant

List every decision and order you want

For each issue state the amount demanded, the amount you say is payable and why. Identify whether the point concerns the meaning of the lease, reasonableness of cost, standard of work, consultation, evidence of expenditure or another rule.

Ask separately for fee reimbursement, a section 20C order and, where relevant, paragraph 5A protection. These are not all the same costs remedy.

Plan the evidence now; build the bundle later

At application stage, identify what will prove each row. The tribunal’s directions will normally shape the final bundle. Keep the source documents unedited and create working copies for pagination later.
