Heading: court, claim number, parties, witness, statement number and exhibits.

1. Identity, role and source of knowledge.
2. Lease and responsibility for the service charge.
3. Demands received and payments made.
4. FTT proceedings and decisions (identify documents; do not re-argue decided issues).
5. Repayment calculation and requests for correction/payment.
6. Defence issues that require factual evidence.
7. Loss/interest calculation where appropriate.
8. Short conclusion and order sought.
9. Statement of truth using the current required wording.

Separate what you observed, what a document records and what you infer. Cross-reference every document consistently.
