1. Heading and introduction
- Court, claim number, parties and witness identity.
- How the witness knows the facts.

2. Lease, payments and parties
- Relevant lease share and payment history.
- Why the named defendant is connected to the money claimed.

3. FTT proceedings
- Applications, decisions and the exact findings relied upon.
- Keep declarations separate from any express payment order.

4. Calculation
- Explain the schedule row by row.
- Identify source documents and resolve credits or rounding.

5. Pre-action chronology
- Requests, calculation sent, response and letter before claim.

6. Response to the defence
- Use one heading for each point in the defence.
- State the factual answer, exhibit, legal submission and calculation effect separately.
- If a signed document is relied upon, address its exact words, context and asserted legal effect.

7. Directions, mediation and conduct
- Record only admissible facts and comply with mediation confidentiality.

8. Remedy
- Principal, interest and costs sought.

9. Statement of truth
- Use the current prescribed wording. Check every paragraph and exhibit before signing.
